NON-PAYMENT OF YOUR INVOICE UP TO 8 HOURS BEFORE YOUR APPOINTMENT WILL RESULT IN AUTO APPOINTMENT CANCELATION BY OUR SYSTEM.

YOUR PHLEBOTOMIST WILL NOT ACCEPT ADD-ON TESTS TO YOUR SCHEDULED ORDER(s) NOR ADD ON A NEW ORDER DURING YOUR VISIT. ALL LAB ORDERS MUST BE SUBMITTED TO QUICK DRAW MOBILE LAB BEFORE YOUR SCHEDULED APPOINTMENT. 

QUICK DRAW IS NOT LIABLE FOR ANY SPECIMENS THAT ARE MIS-HANDLED, LOST, OR SPILLED BY FEDEX/UPS OR THE LAB THAT IS RUNNING YOUR TEST. HOWEVER, WE WILL WORK WITH YOU & THE LAB TO FIGURE OUT THE BEST RESOLUTION IF POSSIBLE.

QUICK DRAW MOBILE LAB HAS THE RIGHT TO REFUSE &/OR DISCHARGE ANYONE FROM SERVICES IF YOUR BEHAVIOR  IS DEEMED INAPPROPRIATE  &/OR FOR ANY OUTSTANDING BALANCE OF ANY SORT.

 

Informed Blood Draw Consent:

I do hereby consent to the drawing of a blood sample for the purpose of medical treatment and/or self-awareness of health. I understand that the risks involved with blood draws include but are not limited to, discomfort, bleeding, soreness and/or redness at the site of collection, feeling light-headed and/or fainting during or shortly after your blood, a pocket of blood underneath the skin (Hematoma) and rarely, Seizures or infection at the site of the blood draw.

I understand and accept that data derived from this blood draw is considered preliminary only and does not constitute any kind of diagnosis. It is my responsibility (as the patient and/or guardian of the patient) to initiate a follow-up examination to confirm results and obtain professional advice and medical treatment. 

I give my consent for Quick Draw Mobile Lab, LLC to deliver my blood/specimen to Lab Corp, Sonora Quest, Fed-Ex, UPS, or any other specialty lab of my choice. If I am receiving Quick Draw Mobile Lab, LLC Point of Care onsite tests, I give Quick Draw Mobile Lab, LLC permission to release results to my primary care provider or myself via Fax or encrypted email at my request or when resulted.

 

 

Payments Policy:

Quick Draw Mobile Labs requires payment (“Payment”) of a fee for use of the Services before services are performed. Specific fees will be outlined on the invoice & website at www.quickdrawlab.com

(a)
General. When you make a payment (a “Transaction”), you expressly authorize us (or our third-party payment processor) to charge you for such Transaction. We may ask you to supply additional information relevant to your Transaction, including your credit card number, the expiration date of your credit card, and your email and postal addresses for billing and notification (such information, “Payment Information”). You represent and warrant that you have the legal right to use all payment method(s) represented by any such Payment Information. When you initiate a Transaction, you authorize us to provide your Payment Information to third parties so we can complete your Transaction and charge your payment method for the type of Transaction you have selected (plus any applicable taxes and other charges). You may need to provide additional information to verify your identity before completing your Transaction (such information is included within the definition of Payment Information).

Canceling Payment. You may cancel a Transaction for a full refund more than 24 hours before a scheduled blood collection appointment. WITHIN 24 HOURS OF YOUR SCHEDULED BLOOD COLLECTION APPOINTMENT, YOUR PURCHASE IS FINAL AND YOU WILL NOT BE ABLE TO RECEIVE A REFUND OF YOUR PAYMENT. If something unexpected happens in the course of completing a Transaction, we reserve the right to cancel your Transaction for any reason; if we cancel your Transaction, we’ll refund any payment you have already remitted to us for such Transaction. To cancel, you can scroll to the bottom of your appointment confirmation that was emailed to you and you will see a button that says “Reschedule/Cancel Here”

No Show Appointments. If your phlebotomist arrives at the address scheduled for your appointment and the patient is not at that location for their visit, we will give a 10-minute grace period for arrival after which your appointment will be considered a no-show and a refund WILL NOT be issued due to inconvenience.

(d)

Late cancelation of appointment. We charge a $35 late cancelation fee if you are canceling your appointment on the same day as your scheduled visit.

(e)

Changing appointment location. You can change your appointment location up to two times before your scheduled visit as long as it’s within an 8-mile radius of your originally scheduled location.
